1. what breed is your dog
2. How old
3. Female/male? Neutered?
4. from a petshop? breeder? adopted?
5. diseases your dog has/had (eg. tick fever, flea allergy, kidney/ liver problems)
6. The timeline of these diseases (when each disease happened).
