So-called 'unathorised structure' has Building Authoriy approval

Has anyone heard of the Building Authority issuing a Section (24)1 demolition order even though the Authority has, many years previously, approved plans for the structure in question?
If so, how did this happen and what was the upshot?

approved plans and given the consent to carry out the work is a different issue.
