In the old days that's what some people did, but I don't think the general population does that nowadays. In the rural areas perhaps. At least the babies don't get diaper rash!

Why all the China examples when I've seen kids taking a dump in the gutter at the side of the road in Shatin (I thought he was going to get run over), peeing on the bus on the way to Sai Kung, and on the MTR - twice, once over the edge of the platform onto the track, and once all over the floor of the train (which went everywhere when the train stopped and started. Talk about going with the flow...)
