1. There is no such list.

2. The various costs involved are standard but none-the-less there are choices.

3. It is sensible business practice for all the involved parties to have the costs confirmed in writing before proceeding!

4. How well they have performed - depends what exactly you mean?!

Ultimately you need to find someone you trust. I suggest you ask for client testimonials.
