Questions re Hiring PH helpers

Hello, we have moved back to Hong Kong after leaving 7 years ago. Our previous PH helper who is living back in the Philippines is going to come work for us again.
I know there have been Changs made since we left.. Questions I have...

1. Can we hire her direct, without agency?
2. How do we get. Form ID407?
3. Has anyone experienced expediting it because of a disabled member of the house?
4. Is it still approx. 4-6 weeks?
