Can we take domestic helpers on vacation with us to help with with babysitting? Is this part of the expected duties and does the DH have a choice in whether to go? What type of insurance is recommended?

I've never heard of asking the Immigration Dept for permission to travel with a helper before... We've traveled several times with our helper and she's never been asked anything or were we advised by the immigration officer.
Iso-no-awabi, was your travel recent? Is this a new rule that has just been put in place?

have taken ours with us a few time and going again this fri - never had problems, you should be fine

we have also travelled with our helper out of hk on vacation...never a prob so far..so should be ok
