How do you pay DH tax on their behalf? I heard that you can pay it in 6 month instalments but who and where do I need to pay it to?

You should have a demand note from immigration, and you would pay it at the immigration department. If you have no demand note you may have paid that to your agent as part of the hire fees, or paid the full amount already as part of the hire process. In filing the immigration forms you have an option of selecting what payment cycle (all at once or in payments). If your helper says she paid it herself ask for proof, it should be an immigration receipt stating that it is for the Levy fee. FYI the Levy is a tax on you for hiring an overseas employee, allegedly for retraining of local unskilled labor. It is not a tax to your helper.

I didnt think helpers were required to pay tax because they do not make the minimum annual salary...

when immigration issue the visa, they will also give you some documents on how to pay, and dates to pay, for me I paid the first six months when they issue the working visa, and I will go pay again after six months. You can choose ways to pay it. You can email immigration they are very helpful on giving informations.
