Not sure about the visa (someone else will know) but he/she won't be allowed to practise the therapy here unless he/she has a working visa - and that doesn't come in single/multiple entry variations.

As long as you realise that it (the therapy in return for 'extra cash') would still constitute a breach of his visa conditions. I think I have some information on the visa question in the office. Back tomorrow.
