Myself and three friends are also going through this palava as we are also hoping to train from HK to Tibet in early June.
One of us is in California a similar problem to your friend visa/permit wise!
We've tried a few travel agents with varying degrees of sucess both on the help front and prices, but the requirement appears to be you need the China visa then hand over photo-copies of passports and China Visa to your travel agent who will obtain the Tibet permit, this could take 10 days i've been warned!
I'll PM you the name of the travel agent we are now using,
