Hmmm...
When I had a North American issued gold visa card before and paid for the rental with it, I was automatically covered with insurance.
Don't know if these offers are still around, but it would be good idea to check with credit crd companies to see if they have any special offers related to spending with their cards on services such as car rentals.

For the US, many (but not all) US-issued Gold cards, and presumably also Platinum, offer car insurance coverage. If you have such a card, and know their website, you can get the answer online from them or send them an e-mail. I asked the card issuer for my HK-based Visa and they just laughed at me, which I took as a "no."
