how to use the word dare?

should i say "i dared not to tell him" or "i did not dare to tell him"?

When used as a intransitive verb in a negative statement, form with the auxillary verb 'do' :

I did not dare to tell him.

When used as a auxilliary verb in a negative statement, form without 'do':

I dared not to tell him.
