To make it short, I would say your I.D. card and document for address proof (e.g. electricity building, bank statement etc). But do we still need library cards these cards? I think you can use I.D. card (after registration with the library) to borrow books now.

Regarding Mighty's response, you can use your Smart Card but, of course, you have to apply to do so:
