Which government office to reclaim MPF?

I understand that in order to reclaim my MPF from HSBC I need to get a witness to chop my declaration that I am leaving Hong Kong. Can anyone tell me the name/address of the government office where I can get this done? I can't seem to locate this info anywhere and I have to wait hours at HSBC to access this highly secret information. Do I need to take anything with me apart from the form and my ID card?

Lower ground floor at the Centre, a government office (declarations or something like that). And there is a HSBC on Queen's Road opposite so you can probably drop it off immediately.
