Has anyone tried to use these - are they reliable 24/7? Or do you have blackouts when satellites are not overhead?

you will have blackouts when inside buildings (and are outside of GSM coverage). Outdoors you can pretty much be wherever you want and you can make calls. No such thing as "no satellite overhead"

If you have ever used a handheld GPS unit you will get a good idea of coverage. Tall buildings (forget it in Central unless you have roof access) or heavy cloud cover limit usage. Great for transatlantic or desert crossings but pricey
