I was born in HK, so I have a "3-star" HK ID card along with my USA Passport. My wife is from PRC and has a work visa in HK. Our baby was just born outside of HK and entered HK using a USA passport. How can I get a HK ID for our baby? Only I have the HK ID; my wfe does not.

Since your child entered HK on an US passport, the first thing you need is to get your child a dependent visa.
Go to the Immigration Department web site, download the relevant forms and instructions.
Read the instructions and file the application.
The process takes a few weeks.
After you get the dependent visa, you can get an ID for your child. (just go to the same website again, and find the instruction for getting ID.)

Hi, I went through the same process with my daughter. Although the required age for and HKID is 11, you can get one for your child at anytime. I found immigration to be very helpful. Online you can book an appointment to obtain the Visa. You should take her with you the first time, don't go on a Monday or Friday and expect at least a one hour wait. Once there you ask if your daughter qualifies for a permanent resident card or if she is entitled to HK citizenship because you are a permanent resident. Once the Visa goes through you can set an appointment online for the ID card, you will have to bring her for that because they will take a picture of her themselves. The whole process will take about 1-2 months, but you will only fill out about 4 forms (online or they can give them to you at immigration) and you will only have to visit the office 4 times (if everything is in order.) Most of the 2 months is just time they take to process everything and you will not have to worry about that, they do a good job.

I guess my follow-up question is this: my wife and I spend half our time in China. After I get 1) the Dependent Visa and 2) the Hong Kong ID card, would it be possible to get her a "Return to HK" card so that she can just travel easily back and forth as I do?

I wasn't born in HK but I have a HK mum with 3 stars on her ID card. So I was able to get a 3-star ID card myself. With 3 stars, you can get the Home Return card from the China Travel Service.
Of course, when your baby gets her ID card (my daughter has one - she's nearly 2 years old) it won't have 3 stars on it. Apparently they give them the 3 stars when they turn 18.
But... I would assume your baby is eligible for permanent ID status... I mean, I got permanent status from my mum and not from living here for 7 years.
If you can read Chinese, then the China Travel Service has all the information on their website about eligibility. Failing that, you can go to Wanchai... or Mongkok branch. I forget which branch. From my experience, CTS will tell you different things but if you go to the correct branch, then they will follow the rules and issue the Home Return card for you, if your baby is eligible.
I tried to get my Home Return card at a few different places, but they all said... you are foreigner, you can't speak Chinese, blah blah blah. Finally I went to Mongkok and they gave it to me immediately without any question! My auntie told me that smaller branches don't know the rules, and so they make up the rules themselves.
Let me know how it goes for you because I haven't yet gotten around to getting a Home Return card for my daughter - but she was born here and has an SAR passport so I imagine the process will be straightforward for her.

Three stars are only available to PR ID holders.
Come to think of it, you'd better go ask immigration.
You are more local than expat. Your are effectively HK Chinese.
Rules for your baby maybe different. It is possible your child maybe eligible for PR right away.

I am born in Hong Kong with permanent I.D card symbol "AZ", non Chinese( no SAR passport). I have no three stars on it?
My friend she has Canadian nationality born in HK and she has three stars but her mother was Chinese.
Is it three stars better than nothing?
