I posted a question regarding name change for my hkg id card in the "practical" thread...but the answers I've gotten have been mixed. So I'll try here.
I want to make sure my married name (not maiden name) is on my sons birth certificate when he is born in a few weeks.
I plan to change my name on my hkg id & passport soon, but have no idea when either will be ready.
Anyway, I'm curious to know the following:
When I check into Matilda hospital on my delivery day, what forms of I.D will they ask to see?
Will I definately need my hkg id with new name change OR new passport with name change, in order to make sure my married name is on my baby's birth cert?
Or will proof alone (papers showing processing) be enough?
I just want to make sure I have whatever papers I need.

I gave birth to my son at the Adventist (not the Matilda) but I think the answer will apply.
I have my passport and ID card under my own name and was unmarried when I gave birth to my son. I have since married.
However, I gave my son my husband's surname from the start and it's quite easily done.
It's not the hospital which will be asking for the paperwork, it will be the Birth Registry. The Matilda should record whatever name you want to give to your baby and then you give the paperwork to the Gov Office when you go to register your baby's birth as that is where you'll get your birth certificate.
If you want that father's name on the birth certificate you have to bring in your marriage certificate if only you are present to register the baby (along with the usual ID cards, passports etc).
In my case where I was unmarried, we both had to go down to register the birth and show the ID cards, passports etc.

I can only comment on one part about registration at Matilda:
If you pre-register with Matilda it will save you a lot of time. I only used my Perm HKID card. If you are not perm, I suggest you bring your passport as well just in case.
For the actual birth certificate, you might want to look at:
http://www.immd.gov.hk/ehtml/bdmreg_1.htm
and maybe call and ask them which name goes on the birth cert:
Maiden (the one you were born with)
Current last name (the one you have at time of birth)
New last name (husband's)
Our birth certificate says:
"Maiden surname and name of mother"
But I did not change my name, so it has my name as it appears on my HKID Card.

My understanding is that Matilda will send a record of the birth to the Birth Registrars office in Admiralty.
When the parents show up to register the birth, they have to provide ID. It is from these IDs that the names are taken for the birth certificate. In other words, get your HKID and passport sorted out before going to register your child's birth.

It's not a problem if you retain your maiden name when your child is born. If you are legally married, it is assumed the child will take the husband's last name.

At Matilda we will register you as on your ID card or Passport (which should have the same name). Pre-registration through the website will help speed up the process but please bring the originals as we are required to see them - that is for both mum and dad.
I hope you can sort it all out before as you will definately have your hands full once baby arrives.
