This is so bizarre, I am usually offering Obstetric advice on this site as an OB in China, But I have a question for BRITISH mums. In the US and Canada and even here in China we take a Vag culture at 36 weeks for BHS to make sure that there is none of this bug lurking in the Vagina. My daughter is pregnant in London, I told her that she should have this done, she is seeinga M/Wifwe who says it is NOT ROUTINE. What is your experience.

Hi Dr. Moreton,
Is your daughter a primipara?
If not, and she is a multip and birthing through a midwife clinic it may no longer be routine.
I can not talk for Britain only Australia where I do know that for a multip with no previous history they are becoming a bit less routine with this test the theory being shorter labours and usually less time with ruptured membranes (so no time to give the appropriate 4 hours of ab cover).
I know you asked for BRITISH mums, sorry but wanted to add my two cents of a happening trend among midwifery clinics in Oz.
Still concerns me though, Grp B strep in a neonate is nasty business indeed, I can understand your concern.

FIRST BABY! Her M/W say it will be routine soon but not now. I have told her to find a private Dr to do it.

Sound advice for your daughter!

I have found out that this test is being introduced as a routine in the UK-NHS, but that some regional authorities have been more active than others in getting it in place.

Only now being introduced? It has been routine in Australia private and public from 36 weeks since about 1993. It would be interesting to see the incidence of Neonatal Grp B step pneumonia (any ideas where I could get that information - you have me thinking now!)

i'm taking that from midwife's comments that if a mother tests positive for the strep B that the newborn can end up with pneumonia? Is this standard practice in HK as well? I delivered at one of the public hospitals but I don't remember ever having a culture such as this taken.

I asked at the public hospital (QMH - where I delivered) and they said they don't test routinely, and there's no need as they monitor the mother's temperature throughout labour, and if the temp rises they give antibiotics as a precaution. I don't know if this is a good or bad way to do it.

I had my first baby in Hong Kong in 2000 and was not tested for Strep B. However my temperature was montiored during labour and when it rose, I was given antibiotics. Second baby was born in the US in 2003 (because of SARS) and I was given a Strep B test. The result was positive so once again I was given antibiotics during labour. Two different procedures, same end result.
