The public hospitals do midwife births, neither of my 2 births in HK had a doctor present.
I also wrote a birthplan, stating no episiotomy (routine here), no lithotomy position (also the norm here), and several other things. All were followed, both were natural births, one with a bit of gas and air, one with nothing.

Just had a complete natural delivery at Queen Mary Hospital, no episiotomy, no drugs/gas, position as requested ie reclining squat. Birth plan was perfectly followed by midwife, although they barely had time to read it ! No doctor present. Just make sure you clearly state what you want/don't want.
