I had one on my finger caused by chewing my nails. I did some research which indicated that you should never try to cut the skin around the nail - instead the advice was to buy some topical antibiotic cream which brings down the swelling and frees up the nail to grow properly.
I followed this and within a day or so the swelling and pain were gone and the nail grew properly.

If problem persists, this guy can help. Small surgery with local anesthesia to remove the ingrown part and in a few days you're like new. He can also "cure" the nailbed to prevent any ingrown nails ever occuring again, if you're prone to that problem...

I've had two surgeries on ingrown toe nails. Basically they cut the toe nail so that it would grow straight and not inside. The first surgery had superb results, the second one gave me crooked and ugly toe nail. Hate my second doctor!!!
I agree, in future, it is better not to cut toe nails in a round shape or cut the edges. Let them grow straight. In addition, do not wear shoes which squeeze your toe nails together. I look at my toe nails now and they are disgusting and I regretted not taking care of them.
