Every 5-6 weeks my back appears to go out. The pain starts as a twinge above one side of my buttock and lower back then builds slowly after about 4/5 days I cannot move. Then two days later it begins to ease. A week after that I can still feel it and am limping. Sometimes the pain then transfers onto the other side of the lower back but is not as chronic. I do not know what to do about it! I have seen physios who seem to make the symptoms last longer. I have had an MRI that was clear. I fell about 2 years ago and landed in the same sort of area, the back problem started about 8 months after this. I am in my late twenties and feel like I have the body of an eighty year old!! Should I just do careful exercise to build up my stomach and back muscles or should I try a different therapy. Any ideas?

You mentioned you have seen physios who only carry out the treatment ordered by a doctor... who rarely the knowledge of soft tissue damage to the lower back. Have you seen a manipulative physio?

Manipulative physios receive additional training. MPs are where physios send their patients to when they can't help them. Also MPs are able to make diagnoses - which ordinary physios can't do - and in some cases are better than sophisticated imaging processes.
MP started in Australia so an Australia GP will be familiar with them. It is now also taught in Canada, the UK and Hong Kong!
I've been to ordinary physios, who can help in the short term, but MPs will teach you effective management of the problem because too often the problem has been going on for a long time and a 100% recovery is not possible.
And an MP will also teach you how to strengthen your core muscles - vital for a healthy back.

I think I might need to see one very soon. I regularly have spine adjustments back in Manila with a chiropractor but it's a few months till my next visit. I had slipped disc a few years back, and once in a while, I get recurring lower back pains. I think I might have done a bad move/position during yoga last week, so now I have a dull ache. Hot packs at night relieve me but can't head back to the gym just yet..

FYI chiropractors never really deal with the root cause of the problem and they are not trained with diagnose or treat soft tissue damage. The "pop" they get is just the release of nitrogen which builds up over time, cracking knuckles is the same thing. This is why you have to see a chiropractor every so often, to release the nitrogen. I have never seen a chiro but can release the nitrogen build-up myself in those joints with a problem.

Manipulative physiotherapy is one field of physiotherapy practice and relates to disorders of the musculoskeletal system through the assessment, diagnosis and physiotherapy management of musculoskeletal conditions.
MPs receive additional training (e.g. postgrad diploma or Msc) and, for many conditions, MPs can make diagnoses not possible with MRIs, etc. Useful to know because too often patients with problems are sent to orthopaedic surgeons and, as we know, surgeons like to cut. BTW, the MP I see has treated a number of doctors, as well as patients send from 'ordinary' physio clinics.
Isn't myofascial release something like Rolfing? It's not something I've looked at for my condition although I did check out chiropractors - which is why I don't go to one.
Of course who one goes to see for lower back problems depends on the condition and the choice of the patient. In my case I don't want a risky operation (poss. result paralysis) when the condition can be managed and don't want to risk my back with someone who has not received medical training. But as I said, that's my choice and perhaps it's not for everyone.

Myofascial release is very much like very controlled accupressure applied directly on a myofascial trigger point. The person doing it has to have a technical understanding of the physiological process and be really finely tuned to the heat and hardness of the trouble spot. Lots of technical explanation missing here of course. (BTW Louise I do not mean to imply that this sounds appropriate at all for the symptoms you describe! I was just wanting to learn about skill / training of MP practitioner.) Louise, I sent you a private msg some time ago did you get it? How is your therapy going?
