Hi,
My Daughter (15 month) had very high fever twice in the past month, and doctor found some bacteria in her urine three times. Although the doctor can not certainly say that she has urine infection, he's doing a serial of tests to find the reason. We just did an ultra-sound yesterday, and he said next one we should do a voiding cystourethrogram (VCUG), which including place a tube into the bladder through the urethra. The procedure sounds quite disturbing to us. I'm wondering if anyone happen to know any ped special in urology that we can consult with? Is this exam really needed, is there any alternative ways?

Hmm, Matilda, what is his training? The reason I ask is that I think my child's pediatric urologist had done three residencies: one in general surgery, one in urology, and then a third one in pediatric urology.

my son who is now almost 12 weeks. He had fever at 6 weeks and the doctor found bacteria in his urine too. He had just gone thru' the VCUG at Princess Margaret Hospital on 26/4. The result is back that he did not have the relux of urine to his kidney. I tot it was aweful too to do this test and I cried before he was brought to the hospital but surprisingly, he was calm. I guess it must be from the 'sleeping' syrup they gave to him 1/2 hour before they inserted the tube. I asked some of my pediatric doctors oversea, they told me this is standard normal test that they will do to any kid who found bateria in thier urine when he/she had high fever. My husband and I decided to let our baby go through such test for the good of him, in case there is really a problem, we can do the next step. It is generally safe to do so although she may find some discomfort in the process. I know because i've gone thro this when i had c-section delivered my son on 14/2.

The test is proper. The renal ultrasound looks for really bad things but the VCUG is normal procedure for infants with urine infections and should be considered in girls under 6 years and any male with a urine infection at any age.
What you are looking for is reflux of urine into the bladder. If you find it, the patient is usually given prophylactic antibiotics and serial VCUG is done until the reflux self resolves. If it doesn't resolve, surgery may be indicated.
You need a urologist only AFTER abnormal test and often not even then. The only time you need a pediatric urologist is when surgery is planned.
