I would see your local doctor or even contact your obestrician for advice. If she is in that much pain, then she needs the tooth out, especially if it's infected or there is a chance it could be. Is it possible that she can express some milk into a bottle and you can freeze some and try to bottle feed for the 12-24 hours afterwards. I suffer from migraines and when I was breastfeeding and had the migraine I would give my baby a feed and then take my medication directly after. The doctor said such a small amount passes through the breastmilk there is no problem. Look at how babies were given birth years ago and even today occasionally, women were put completely under in a general. Definately call your GP or Ob to get the good, correct advice. Good luck, there is nothing worse than a tooth ache, especially when caring for a newborn. I feel for her.

As the daughter of 2 dentists and having worked as a chairside nurse for both of them I can let you know that I've never seen them choose to not treat a pregnant or lactating woman with the excepion of xrays. The LA used is metabolised so quickly and wouldn't be a prob for the baby. Your wife can't be in pain for the next weeks or months. It's not realistic. SHe has to look after a baby and herself and deal with the associated sleep deprivation and to add pain on top of that would just be cruel.....
See another dentist or even consider hypnosis or accupuncture for anaesthesia.
