HKD 4,500? The highest fee I know of is a surgeon charging HKD 2,500 for a pair which costs a small fraction of that to him. I suppose he was charging for his expert advice, namely you should pay him lots of money.
Orhotics don't cost that much in HK and you only need about 30 minutes training to sell or offer advice on them. Another 30 minutes if you want to sound plausibly professional. There is probably more training involved in cooking a quarter pounder in a MacD.
Almost all "re-sellers" get them from one of several sources in China and the odd ones from the US. Orthopedic surgeons will have the highest markups for the same products from the same manufacturer. There are a number of chiropractors in Central who offer these services and they typically charge about HKD 1,600 to 2,000 per pair and an additional 50% for the second when ordered together. A direct re-seller will charge you less, say HKD 1,300. Apart from the US made ones, they are almost all made by the same few people in China.
My experience with orthotics is that it's a bit of hit or miss, like most things medical. If you have a problem, this could be one of the solutions or not. You don't know till you have tried. Just find out more first.

I have suffered from heal pain, plantar faciaitis, I wear orthotic inserts custom (made by Douglas Horne) in my trainers and it seems to do the trick, I did discuss surgical options when the pain was really bad, but the outcome of that is not always 100%, I wear Dr Kong sandals in the summer,with plenty of support for my feet, can't wear pretty high heels at all, would rather put up with the less attractive but more functional shoe or sandal, and no pain.
