Does anyone know the function of this test? When the doctor asks you to drop your pants, he cups your testicles and asks you to cough?

By coughing you raise the pressure in the abdomen, which will push out a hernia through a weakness of the abdominal wall in the lower abdomen. The doctor can than either see or feel such a hernia (if there is one).

At a physical examination (check-up) the doctor may also feel the testicles for lumps and bumps (cupping the testicles).

I see... but why is it necessary to cup the balls if the hernia is in the lower abdomen?
